Handlebar clamps are to be tightened with no gaps on top. All stems have max torque specs etched onto the stem body. Note that max torque specs are just that, a maximum rating. For most purposes, 5-6nm should be adequate.
There is a 16.8mm difference in width between the narrow and wide settings, measuring 55.5mm and 72.3mm respectively. In the narrow setting, you'll have more compliance in the feel of the bar, while the wide setting will provide a firmer, more rigid feeling. It's all down to personal preference and how you prefer your bike to feel.
